Patient Registration Forms

In preparation for your first visit at our office, you may review and fill out the following forms before your visit:

• Patient Registration Form
• Health History Form
• Privacy Policy Acknowledgement
• Authorization For Disclosure
• HIPAA Form
• Medical Services Financial Agreement Form
• Release of Medical Records Form

Please fill out each form as completely as possible and bring them to your appointment.  Also, please review our Notice of Privacy Practices document before your visit.

New complete eye exam appointments usually take 1-2 hours.  Glaucoma and Uveitis new appointments take about 2 hours, but are highly dependent on required diagnostic steps.  You will likely have your pupils dilated, so you may wish to bring a driver or make appropriate plans.  Dilation drops usually take 3-4 hours to wear off.
